Policies & Good Faith Estimate


Scheduling 

  • All sessions are by appointment only. You can email me at info@lameesaisami.com to get set up with an appointment.

  • If you need to cancel or reschedule your appointment, kindly let me know at least 24 hours in advance of your scheduled session time to avoid being charged the session fee.

  • I offer 15-20 min free Zoom consultations. During this time we will talk about what you’re looking for and determine if we are a mutual fit. I will also answer any questions you might have about the therapy process and we can discuss logistics for getting started.

Billing

  • Your fee will depend on which type of session(s) you choose, as outlined on my offerings page. Most typically, individual sessions are $110 per hour. I do have limited sliding scale slots available and group sessions offer more access to affordability.

  • You will never be charged more than what we agree upon and if there are any changes in my rate, you will be notified verbally and in writing well ahead of the change taking place.

  • *Having a reliable credit or debit card on file is required before we can begin working together. 

  • Square, Zelle, HSA/FSA  debit cards, and all major credit cards are accepted for payment. 

  • You will be invoiced on the day of your appointment and payment is required before offerings can be provided. 

  • Insurance: For all insurance plans, I am considered an Out of Network provider. However, my services may be covered in full or in part by your health insurance or employee benefit plan. Please know that you will be responsible for the fee at the time of our sessions regardless, but some of the fee may then be reimbursed to you through your insurance company if your plan has out-of-network benefits.

  • Questions to ask insurance company re: out of network benefits:

    • Do I have coverage to see a behavioral health provider who is “out-of-network”? 

    • Do I have “outpatient mental health benefits”?

      *If the answer is YES for both questions, then also ask:

    • Do I need authorization for the visit(s), and if so, how do I get authorization?

    • Do I have an out-of-pocket “deductible” (a sum you’ll have to pay before your company will begin reimbursing you)? If so, what’s the amount?

    • How much of my deductible have I met for the current year?

    • What percentage of the provider’s actual fee does my plan cover?

      NOTE: Some companies limit the amount they will reimburse for a service to a percentage of a capped fee that they determine is “reasonable and customary.”

Other

  • Your confidentiality and the sanctity of our therapeutic container is important.
    It is my policy not to approach you if we happen to see one another out in public. You are welcome to approach me, however, with the understanding that this could compromise your anonymity.

Good Faith Estimate

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

  • Make sure your clinicians gives you a Good Faith Estimate in writing at least 1 business day before your service. You can also ask your clinicians, and any other provider you choose, for a Good Faith Estimate before you schedule an item or services.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
